Attention BadgerCare Plus CORE Members! If you enrolled with MHS in April – You must get a Physical Exam by the end of the year.

As a BadgerCare Plus Core member you are required to have a complete physical this year. If you enrolled with MHS in April then you must complete a physical exam by December 31, 2009 to continue your 2010 health coverage. If you do not have a physical exam by the end of the year, your coverage will end. You will also not be allowed to re-enroll for 6 months.

A visit to the doctor’s office for a sickness is not enough. You must get a complete physical exam.
