We have a special program that helps you get care. The program connects you to community groups that can help. It’s called MemberConnections™. A MemberConnections team member can talk to you on the phone. Just call them at (888) 713-6180 and ask for Member Connections, (TDD/TTY (800) 446-6136). They will send you information and can visit your home. We will be glad to talk to you about:

• How to choose a doctor (PCP)
• How to change doctors
• The healthcare you get at MHS/NHP
• How to use MHS/NHP services
• How to get medical advice when you cannot see the doctor
• Emergency care and urgent care
• How to live a healthy life
• How to get shots and health screenings
• Other healthcare problems you may have

MemberConnections representatives can refer you to community social service programs that can help you with food, housing and clothing.

CONNECTIONS Plus Phone Program

CONNECTIONS Plus is a cell phone program for members who are pregnant or have other serious health-related issues and are in need of a phone. To qualify for this program, members must be high-risk or have special needs. When you qualify, a MemberConnections Representative comes to your home. Our members are given a free cell phone. This cell phone has limited use. Members may use this cell phone to call:

• Health plan case manager
• PCP
• OBGYN
• NurseWise
• 911
