Notice of Pregnancy
MHS Health reimburses practitioners for completion of the Notice of Pregnancy form (PDF). One notice is accepted and one payment is given per member. MHS Health will pay $75 for 1st trimester notice, $50 for 2nd trimester notice and $25 for 3rd trimester notice.
Fluoride Varnish Application
Application of topical fluoride varnish by a physician or other qualified healthcare professional can be billed under code 99188. Providers may also submit Health Check and office visit procedure codes in addition to procedure code 99188, when appropriate.
Periodic Oral Evaluations
Physicians may perform periodic oral evaluations on established patients and bill MHS Health using procedure code D0120. Providers may also submit Health Check and office visit procedure codes in addition to procedure code D0120, when appropriate.
Body Mass Index (BMI)
MHS Health reimburses $10 for CPT II code 3008F for members who are seen for a routine/scheduled Health Check or office visit. Reporting of 3008F is limited to nurse midwives, physician assistants, nurse practitioners and physicians.
1 Incentives - paid outside of a claim, checks are mailed to the provider quarterly.
2 Enhanced Rates - paid as part of a claim.
3 Fee-For Service - commonly missed fee-for-service dollars, rates may vary depending on provider contract.
