MedPharm Transition to Centene Pharmacy Services for Utilization Management and Updated Outpatient Medicaid Prior Authorization Form

Updated 10/31/2024: On December 2, 2024, MHS Health Wisconsin Medicaid MedPharm work will transition to the Centene Pharmacy Services (CPS) Utilization Management (UM) MedPharm Shared Services Team. 

• MedPharm prior authorizations (PAs) will route to the Compass-MHK system via a dedicated fax line. 
• Transition to the CPS UM Shared Services Model and Compass-MHK will be seamless for members. 

Note: The Outpatient Medicaid Prior Authorization Form has been updated to reflect the new Biopharmacy fax number. Providers should use a new fax number for Biopharmacy.

• The new Biopharmacy fax number is 1-833-893-1519.
