It’s time to take control of your health – and this is the place to start. Your new health insurance plan offers comprehensive medical care and benefits. It also includes valuable programs, educational tools and support.

This page will help you understand your health plan. It has all the details you need to help manage your health. You can also find tools to take action.

Your next steps with your new plan:

  1. Create an online account. You can manage your health benefits, change your primary care provider, contact a plan representative, view claims and more.
  2. Complete a Health Assessment. This is available on your online account, or call 1-888-713-6180 (TDD/TTY: 1-800-947-3529).
  3. If pregnant, complete a Pregnancy form. This is available on your online account, or call 1-888-713-6180 (TDD/TTY: 1-800-947-3529).
  4. Choose a provider. You can view the providers available to you on our online directory. Choose or change your provider through your online account, or call 1-888-713-6180 (TDD/TTY: 1-800-947-3529).

About Your ForwardHealth Member ID Card

Your ForwardHealth card is a plastic card with your name on it. It also has a 10-digit number and a magnetic stripe. Always carry your ForwardHealth card with you. Show it every time you go to the provider or hospital and every time you get a prescription filled. You may have problems getting health care or prescriptions if you do not have your card with you. Also, bring any other health insurance cards you may have. This could include any ID card from other service providers.
