If you want to take control of your health and wellness, this is the place to start. Your health benefits offer medical, dental, and vision care. And, as a member of MHS Health, you also get access to special programs and services, learning tools, and people who will help you with any type of support you need.

The information on our website will help you better understand how to make the most of your health benefits and we're here to help you every step of the way.

Explore our online member portals anywhere you are — any time day or night. They're free, safe, and secure!

In the  Member Porrtals, you can:

  • Check your health benefits
  • Update your personal information
  • View your claims
  • See your My Health Pays® rewards balance
  • Change your primary care provider
  • Send us a message view claims
  • And more
     

You can access the portals in two ways. 

  1. From home from your computer
    Create an account or log in through our website. 

  2. With your smart phone or tablet 
    Stay connected with the Health Insurance Portal mobile app. It's free to download in the App Store (Apple) or get it on Google Play (Android).

Complete your health screening. 

A health screening asks about your health habits, medical history,  lifestyle, and social needs so we can give you more personalized support and tell you about our programs and services.

Medicaid SSI members must complete a screening every year. Complete your health screening now.

Choose a provider.

You can view and learn more about the providers available to you in our Find a Provider online directory. You can view the provider assigned to you and/or change your current provider through the secure Member Portal available on our website or the Health Insurance Portal mobile app, or call Member Services at 1-888-713-6180 (TTY: 711).

Pregnant? Let us know right away.

Whether you're in your first trimester or last, fill out the Notice of Pregnancy form in your online account, or call 1-888-713-6180 (TTY: 711). Then, head over to our Pregnancy Services page to learn more about our Start Smart for Your Baby® program and how you can start earning rewards dollars.

Your ForwardHealth Member ID Card

Your ForwardHealth card is a plastic card with your name and ID number. Carry your card with you and show it every time you go to the provider or hospital and every time you get a prescription filled.  Also, bring any other health insurance cards you may have. This could include any ID card from other service providers. You will not get a separate ID card from MHS Health Wisconsin.

Call ForwardHealth Member Services at 1-800-362-3002 if you have questions or if your card is lost, damaged, or stolen.
